Looking for unknown link

I have created a form page and when I click "send" I get this error:

ERROR_DEFAULT_404FILE NOT FOUND: contact-us/index.php
URI:/~bellefar/contact-us/index.php

I am unable to find this "index" page. Also, when I post a link in the redirect URL form it continues to try to find this link.
I am looking for assistance on how to successfully send the information on "contact us" form page.

You had activated SEF and it is not properly setup.
I need more details about what you're doing to be able to help you.
What SEF extension are you using?
Did you renamed htaccess.txt to .htaccess?
Do you have mod-rewrite activated on your server?

Basically this is a Joomla problem, not connected with aiContactSafe.
If you properly setup SEF in Joomla aiContactSafe will work. You just need to install the correct plugins that are making the connection between Joomla and the SEF extension you're using if you are not using the native solution.
