Hi there,
I'm just setting up with your contact form, and everything seems to be working well so far.
The only problem is that no field shows up for attachments. I saw the earlier posts about file types. So I went into my Global Configurations and made sure my image file types were the same as the ones shown in the Control Panel of aiContactSafe.
Is there something else I should be doing?
My form is currently here:

Did you added a new "File" field in the "Fields" section ?
