I had 3 ai_contactsafe forms on my website www.espiritorock.com but I'm experiencing some trouble. I disabled two of them and only left the contact form. I enable on the configuration panel : Name - email - subject - adicional info - send to sender

When I view the contact form on the web I only view Subject - adicional info - send to sender and captcha (that I had disabled previously) www.espiritorock.com/index.php?option=co...6&Itemid=216&lang=pt

Do I need to reinstall the component?

Your message is not very clear.
You can control which fields to display and on what position from the profile.
I suggest reading the instructions ( click the "Help" button from the top right toolbar ) of how to set it's parameters.

I have enabled the fields: Name - email - subject - adicional info - send to sender

I don't know why I can't see the fields I have enabled and am seeing the captcha that is disabled.

Now I understand what you mean.
You problem is most probably generated by the fact that you have some field parameters with unclosed quotes ( " ) or apostrophes ( ' ).

ok thanks, that solved the problem.

One other thing. My form looks funny I have attached a picture, I tried many times to get it right... I tried to enable/disable Name after field but it doesn't solve the issue.

Go into the profile you're using, click "Edit CSS" and replace "div#aiContactSafe_mainbody_1" with "div#aiContactSafe_mainbody_6" ( notice you have to replace "1" with "6" ) everywhere it appears.
